Managing the transition from inpatient to outpatient care is among the most important inflection points in a patient’s care journey. As behavioral healthcare providers, we must do all we can to support patients during this vulnerable phase. Support includes working as a collaborative team with family members and peer specialists, cultivating human connection, building lasting bridges, and creating the opportunity to connect outpatient providers prior to discharge to develop rapport.
